What type of information we have:

The type of information that we collect, and hold is personal information, that can be
used to identify a living person, consisting of personal identifiers, contacts and
characteristics that consists of:

  • Name
  • Addresses
  • Telephone numbers
  • email addresses
  • NHS numbers
  • Medical history
  • Consultation notes

How we get the information and why we do we have it:

We collect personal data on a legitimate interest’s basis, to be able to provide patient
care, the data we receive originates from GP practices within Thrive and referral
sources form the list below:

  • Mid & South Essex NHSFT
  • Nuffield Hospital Brentwood
  • Connect SWE
  • Ramsay Essex
  • Spire Health Care Essex
  • Springfield Essex

Patients have the right to withdraw consent of the sharing of their personal information
by informing their GP practice or referrer.

What we do with the information:

We share the personal information that we hold and collect back with patients GP
practices or to third party sources when there is a need for us to refer patients on for
treatment.

How we store your information Your data protection:

The personal information that we hold and collect is held on a centrally hosted clinical
computer system call SystmOne. Once care is completed, we discharge patients from
our care which removes patients from our accessibility. Referrals that are received are
stored on a secure email address and are transferred to patients records on SystmOne
once processed.

Under the General Data Protection Regulation (GDPR), the lawful bases we rely on for
processing this information are: Legitimate basis.

This means that you can remove your consent at any time. You can do this by contacting
your GP practice of referrer.

We have a legitimate interest.

Rights including:

  • Your right of access – You have the right to ask us for copies of your personal
    information.
  • Your right to rectification – You have the right to ask us to rectify information
    you think is inaccurate. You also have the right to ask us to complete information
    you think is incomplete.
  • Your right to erasure – You have the right to ask us to erase your personal
    information in certain circumstances.
  • Your right to restriction of processing – You have the right to ask us to
    restrict the processing of your information in certain circumstances.Yo
  • Your right to object to processing – You have the the right to object to the
    processing of your personal data in certain circumstances.
  • Your right to data portability – You have the right to ask that we transfer the
    information you gave us to another organisation, or to you, in certain
    circumstances.

You are not required to pay any charge for exercising your rights. If you make a request,
we have one month to respond to you.
